4/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted JaCaeri upon arrival to the home setting. Upon arrival to the home, Ms. Cosby reported that JaCaeri had a good day and that she will continue to provide QMHP-C with his behaviors as they continue to appear. Mother reported that he continues to present negative behaviors such as him fighting with his siblings as well as him utilizing profanity when things do not go his way. 2. Behavior: JaCarei appeared be frustrated at times throughout the session as he would ball his arms up when his behaviors were being discussed about managing self-control. 1.Intervention: QMHP-C engaged JaCaeri and Ms. Cosby in a family counseling session at the home. QMHP-C engaged JaCaeri and Ms. Cosby in a discussion to allow them to express feelings and moods regarding this session. It appeared that Ms. Cosby has several areas concerning JaCaeri’s behavior to discuss with counselor. QMHP-C identified incongruent body language with their communication. QMHP-C explained the effectiveness of addressing negative behaviors immediately. QMHP-C challenged mother to implement consistent parenting skills. QMHP-C provided supportive counseling throughout this session. 2. Intervention: QMHP-C engaged JaCaeri’s grandmother and in discussion and she was asked to reflect on the consequences that JaCaeri received based on the choices he has made. QMHP-C provided Ms. Cosby with alternative consequences for JaCaeri based on the choices he has made. QMHP-C assisted JaCaeri’s mother with a written list of consequences that matches choices that JaCaeri tends to make consistently. QMHP-C emphasized the importance of matching consequences with choices and being consistent while doing so and expressed to her that if she implements consequences on a daily basis when JaCaeri displays negative behavior then the relationship between Ms. Cosby and him would not be the way that it is as well as the boundaries between the two of them. 1. Response: JaCaeri appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. They both shared positive feedback regarding the topics being discussed. JaCaeri and Ms. Cosby lacked the ability to maintain congruent body language and verbal language. Ms. Cosby has made several attempts to address JaCaeri’s negative behavior immediately, due to Ms. Cosby’s constant frustration with JaCaeri’s behavior; she has not done so on a consistent basis. 2. Response: Ms. Cosby indicated to QMHP-C that when it comes to her kids she does not play with them. She stated to QMHP-C that she does discipline them when they are together at her house. They both recognize triggers but are unable to respond to them appropriately. JaCaeri struggled to name 3 good and bad choices; however, JaCaeri’s grandmother was able to assist JaCaeri with identifying some bad choices that he has made within the last day.
